Here is a JPEG version at 300 dpi. There are 3 A4-sized pages stacked there. Note that you need to print each of the first two pages twice, and that this template is made for Nikon flashes.
Thick cardboard is a must for these, but can be really tricky to feed properly in a standard inkjet printer. Large size printers are better for that purpose, as they are actually meant to take cardboard...
